  1. Sign in to My Account
  2. Attach a clear electronic copy of your official document to a message in Message Centre

In your request, include:

  • Your Person ID number and an additional identifier (e.g., date of birth, address)
  • A clear copy of your marriage certificate or legal change-of-name document

Another way to tell us of your name change

If you’re an active or inactive member, sign in to My Account and update your personal profile information.


Eligibility

If you’re an active, inactive or retired member, you can change your last name by sending us a document with your new name.

If you’re a limited member, contact us to let us know your name has changed.

Required information
  • Your Person ID number and an additional identifier (e.g., date of birth, address)
  • A copy of your marriage certificate or legal change-of-name document
  • For retired members only: a clear copy of your current identification
